Introduction/Background Eastern Cooperative Oncology Group performance status (ECOG-PS) is the most widely used measurement of patient's ability to perform daily activities. Although the prognosis and effectiveness on therapies based on the PS are assigned to patients, the reliability is still questionable and manifests great variability among the doctors and patients. The objective is to conduct a prospective randomized triple blinded study on 127 gynecologic oncology patients in which, three gynecologic oncology fellows (observers) grade them one by one without having knowledge of each other´s score results.

Methodology The patients were questioned by the observers in a randomized sequential manner. Linguistic validation was also conducted by our group for Turkish version of ECOG performance scale.

Results Kendall's W coefficient was found out to be 0.043. Overall agreement was 74.42% in terms of ECOG PS between the three observers and the average agreement according to ECOG scores; 0, 1, 2 and 3 were 76.16%, 68.3%, 61.6% and 91.63% respectively. There were no patients that were scored as 4. The agreement was the least variable for the score 3.

Conclusion Our results showed that ECOC performance scale in gynecologic oncology patients having good interobserver variability and this questionnaire can be reliably used in gynecologic oncology patients in the assessment of physical health status.
