663 Mature teratoma of the ovarian cancer (study of 22 cases)

Abstract

Introduction/Background Mature ovarian teratomas represent 20% of benign ovarian tumors.

Methodology This work allows us to discuss the diagnosis of mature teratoma in the face of any pelvic pain and highlights the place of surgery in the treatment in 22 cases collected within the Gynecology Department, Chu Ibn Rochd Casablanca, between 2018 and 2021.

Results During the period of our study, 22 cases of mature ovarian teratomas were included. The age of our patients is between 16 and 65 years old with an average age of 42 years old. Pelvic pain was the main warning sign; the diagnostic strategy was based on ultrasound and pathological examination of the surgical specimen. The treatment was exclusively surgical. 4 patients benefited from laparoscopic treatment and 18 patients from laparotomy. No postoperative complications were reported. Histological examination revealed malignant degeneration in one patient.

Conclusion Mature teratomas present a fairly common pathology. They are usually revealed by gravity-type pelvic pain. Clinical examination often finds a palpable mass. Ultrasound is the diagnostic tool of reference. CT and MRI are performed as second intention. Pathological examination is essential to make a definitive diagnosis and to look for malignant degeneration.

Conservative treatment is the rule for all women, especially women wanting to become pregnant. Laparoscopic cystectomy is effective in the management of benign teratomas. The prognosis is excellent.

Thanks to an adequate diagnosis and laparoscopic cystectomy, the preservation of ovarian functions remains good in most cases. Histology remains essential.
