Abstract

Introduction/Background The development of the extraperitoneal space uses to be the most challenging step to perform an extraperitoneal para-aortic lymphadenectomy. Few papers describe the technique, but none explain step by step how to develop this virtual space effectively. The success on developing it will influence the rest of the surgical procedure.

Methodology In the video a step by step technique is carefully explained, adding tips a tricks on haw to avoid peritoneal rupture of development of wrong spaces that may difficult the dissection.

Additionally graphs and pictures have been added to the video to illustrate the technique.

Results The crucial steps include diagnostic: laparoscopy to assess spread of the disease, development of the extraperitoneal layer under direct vision, first trocar placement and insulation of the extrapneumoperitoneum, accessory tracers placement and dissection of the para-aortic space.

Conclusion A systematic technique can improve the success rate of exztraperitoneal para-aortic lymphadenectomy.
