EP444 Endogenous intoxication as a symptom of advanced gynecological cancer

Introduction/Background Development of endogenous toxicosis in cancer patients is facilitated by intensification of catabolic processes, activation of proteolysis, accumulation of biomolecule oxidation products, together with disrupted detoxification and excretion. The purpose of the study was to evaluate endogenous toxicosis in patients with gynecological cancers of different stages.

Methodology The blood of 136 patients aged 32–76 years was studied: ovarian cancer, OC (stage I–II) n=10, ascitic OC, AsOC (stage III) n=18, advanced OC, AdOC (stage IV) n=18, non-metastatic cervical cancer, CC (stage III) n=29, metastatic CC, MCC (stage III–IV) n=27, primary vulvar cancer, VC (stage III) n=34. 20 non-cancer women of comparable age were controls. Medium weight molecules (MWM) at wavelengths of 254 nm and 280 nm were determined, as well as total albumin concentration (TAC), effective albumin concentration (EAC), albumin binding capacity (ABC) - EAC/TAC • 100%, and intoxication coefficient (IC) - MWM254/EAC • 1000.

Results EAC was decreased in all groups compared to controls, with maximal decrease in AsOC (48.1%) and minimal in OC (I–II st) - 20.3%. In CC and VC, EAC decrease was 33–40.7%. ABC decreased in AsOC by 37.3%, in OC (I–II st) by 14.8%, in OC by 25.5–35.1%, CC and VC - 27.7–36%. The highest concentrations of both MWM fractions (254 nm and 280 nm) were noted in AsOC (exceeded the norm by 16.8% and 77.2%) and OC I–II st (exceeded the norm by 25.4% and 74.8%). Elevated MWM280 levels were observed in MCC - 26.7%, and AdOC - 49%. IC in AdOC exceeded controls by 2.1 times, in AsOC by 2.3 times. In other groups of patients, IC was 28.5–90.5% higher than in controls (on average by 53.5%).

Conclusion Patients with advanced cancers showed the greatest abnormalities in the detoxification system associated with the accumulation of endogenous products and dysfunction of natural detoxification and biotransformation systems.
